Neath Port Talbot Council’s new Homecare Rapid Response Team (HRRT) is gearing up for its first Christmas.
The team will be out and about in the weeks leading up to Christmas to support people so that they can spend the festive season in their own homes. Just as importantly it is also vital that NPT Council plays its full part with NHS colleagues to prevent 'bed blocking' which can be a major problem at this time of year.
December is always a busy time for the health and care sectors, as hospitals work hard to get people home for Christmas. The Council formed the HRRT earlier this year to provide short term, intensive support to people in their own homes.
The team does this by operating a fast response personal care service that is available seven days a week. They provide a ‘step down’ service for people who are medically fit to leave hospital, but need support at home, and personal care and support at home to people at risk of being admitted to hospital.
The service can be provided for up to three weeks whilst longer term arrangements are put into place. Since it was formed in July the HRRT has supported over 150 people.
For the HRRT and the Council’s regular Homecare Service Team, Christmas Day is just another working day where people are cared for at home with their family around them.
Rapid Response Team member, Eirlys Ryan, who is one of the staff who will be working over Christmas said:
“This job can be difficult at times, but it is also very rewarding, especially at this time of year when you are making calls to people who, without the support of the team, would have to spend Christmas in hospital.
“Although we provide a short term service, you still get to know the people you support and this personal sense of care makes the job very fulfilling."
Council Leader, Ali Thomas OBE said:
“We recognise that the Council has a part to play in working with the Health Board to ensure that older people are not in hospital unnecessarily. This year we set up the Homecare Rapid Response Team in a bid to support more people in their homes and help reduce the pressures placed on local health services.
“Given the choice, most people would want to spend the festive season in their own homes and the dedication of staff such as those in the HRRT and our regular Homecare Service means that the delivery of care doesn’t stop for Christmas. Their commitment to their work and the people they care for is commendable.
“Of course there are many other workers that we should acknowledge, both within the Council and in other vital sectors such as Health, the emergency services and charitable organisations who, for the good of others, will also be treating Christmas as just another working day.”

TYCGC yn barod am gyfnod y Nadolig
Mae Tîm Ymateb Cyflym Gofal Cartref newydd (TYCGC) Port Talbot yn paratoi ar gyfer ei Nadolig cyntaf.
Bydd y tîm ar grwydr yn ystod yr wythnosau cyn y Nadolig i gefnogi pobl fel eu bod nhw'n gallu treulio gwyliau'r Nadolig yn eu cartrefi eu hunain. Mae yr un mor bwysig hefyd bod Cyngor Castell-nedd Port Talbot yn chwarae rhan lawn gyda chydweithwyr y GIG i atal "blocio gwelyau", sy'n gallu bod yn broblem enfawr ar yr adeg hon o'r flwyddyn.
Mae mis Rhagfyr bob amser yn adeg brysur i sectorau iechyd a gofal, gan fod ysbytai'n gweithio'n galed i sicrhau y gall pobl fynd adref ar gyfer y Nadolig. Crëodd y cyngor y Tîm Ymateb Cyflym Gofal Cartref yn gynharach eleni er mwyn darparu cefnogaeth ddwys, tymor byr i bobl yn eu cartrefi eu hunain.
Mae'r tîm yn gwneud hwn drwy weithredu gwasanaeth gofal personol sy'n ymateb yn gyflym sydd ar gael saith niwrnod yr wythnos. Mae'r tîm yn gwneud hyn mewn dwy ffordd allweddol - darparu gwasanaeth ‘camu i lawr’ ar gyfer pobl sy'n feddygol iach i adael yr ysbyty ond y mae angen cefnogaeth arnynt gartref, a darparu gofal a chefnogaeth bersonol gartref i bobl sydd mewn perygl o gael eu hanfon i'r ysbyty.
Gellir darparu'r gwasanaeth am hyd at dair wythnos wrth i drefniadau tymor hwy gael eu gwneud. Ers iddo gael ei sefydlu ym mis Gorffennaf, mae'r Tîm Ymateb Cyflym Gofal Cartref wedi cefnogi 150 o bobl.
Ar gyfer y TYCGC a thîm gwasanaeth gofal cartref arferol y cyngor, mae dydd Nadolig yn ddiwrnod gwaith arall lle mae pobl yn derbyn gofal yn eu cartrefi gyda'u teuluoedd.
Meddai aelod o staff y Tîm Ymateb Cyflym, Eirlys Ryan, a fydd yn gweithio dros y Nadolig,
"Mae'r swydd hon yn gallu bod yn galed weithiau, ond mae hefyd yn swydd gwerth chweil yn enwedig ar yr adeg hon o'r flwyddyn pan rydych chi'n galw i weld pobl a fyddai'n gorfod treulio'r Nadolig yn yr ysbyty oni bai am y tîm.
"Er ein bod ni'n darparu gwasanaeth tymor byr, rydych chi'n dal i ddod i adnabod y bobl rydych chi'n eu cefnogi ac mae'r ymdeimlad personol hwn o ofal y swydd yn rhoi cryn foddhad".
Meddai Ali Thomas OBE, Arweinydd y Cyngor,
"Rydyn ni'n cydnabod bod gan y cyngor ran i'w chwarae wrth weithio gyda'r Bwrdd Iechyd i sicrhau nad yw pobl hyn yn yr ysbyty'n ddiangen. Eleni, sefydlon ni'r Tîm Ymateb Cyflym Gofal Cartref i gefnogi mwy o bobl yn eu cartref a helpu i leihau'r pwysau ar wasanaethau iechyd lleol.
"O gael dewis, byddai'r rhan fwyaf o bobl eisiau treulio gwyliau'r Nadolig yn eu cartrefi eu hunain ac mae ymroddiad staff fel y rheiny yn y TYCGC a'n gwasanaeth cartref gofal yn golygu nad yw gwasanaethau gofal yn dod i ben dros y Nadolig. Mae eu hymrwymiad i'w gwaith a'r bobl maent yn gofalu amdanynt yn glodwiw.
"Wrth gwrs, mae llawer o weithwyr eraill y dylen ni eu cydnabod, yn y cyngor ac mewn sectorau hollbwysig eraill fel iechyd, y gwasanaethau brys a sefydliadau elusennol a fydd, er lles eraill, yn ystyried y Nadolig fel diwrnod gwaith arferol."
