A pioneering community support service for vulnerable residents has been expanded across the whole of Neath Port Talbot.
Neath Port Talbot Council’s Local Area Coordinators work alongside people who are at risk of reaching a crisis in their life or help people to recover from a crisis. The Coordinators work with people and their families who need support due to a range of different circumstances such as being elderly, having a disability or mental health issues.
The wide-ranging remit of a Coordinator includes:
• Providing people and their families with information on support services available in the community.
• Supporting people on a one to one basis who may be facing difficult times to enable them to pursue a better life.
• Building strong partnerships with community groups, services and organisations to help develop their ability to meet the needs of local residents.
The service was previously only available in a limited number of areas across Neath Port Talbot but it has now been made accessible to all residents. In total, six Local Area Coordinators cover the communities in the county borough.
Councillor Peter Richards, Cabinet Member for Adult Social Care and Health, said:
“Our Coordinators can help people to help themselves through using their wealth of local knowledge and connecting people to existing resources in the community. We want to work alongside people to increase their independence and personal resilience so they can lead the best life possible.
“Local Area Coordinators are a key element to prevent, delay and reduce the demand of people needing costly social care and health services.”
Emma Jones, one of the six Local Area Coordinators in Neath Port Talbot, said:
“A large part of my role is connecting people to people. Many of the people I meet may be isolated and feeling lonely. I work to bring people together in places where there is a good sense of community, where people support each other and build long trusting relationships.
“I feel privileged to be a Local Area Coordinator because I can help develop communities where people want to live, feel safe and be strongly connected.”

Gwasanaeth cefnogi cymunedol yn cael ei ehangu ledled Castell-nedd Port Talbot
Ehangwyd gwasanaeth cefnogi cymunedol arloesol ar gyfer preswylwyr diamddiffyn ledled Castell-nedd Port Talbot gyfan.
Mae Cydlynwyr Ardaloedd Lleol Cyngor Castell-nedd Port Talbot yn cydweithio â phobl sydd mewn perygl o gyrraedd argyfwng yn eu bywyd neu'n helpu pobl i ddod dros argyfwng. Bydd y cydlynwyr yn gweithio gyda phobl a theuluoedd y mae angen cefnogaeth arnynt o ganlyniad i amrywiaeth o amgylchiadau gwahanol megis bod yn oedrannus neu'n anabl, neu broblemau iechyd meddwl.
Mae dyletswyddau amrywiol cydlynydd yn cynnwys:
• Darparu gwybodaeth am wasanaethau cefnogi sydd ar gael yn y gymuned i bobl a'u teuluoedd.
• Cefnogi pobl ar sail un i un a all fod yn wynebu adegau anodd er mwyn eu galluogi i gael bywyd gwell.
• Adeiladu partneriaethau cryf â grwpiau cymunedol, gwasanaethau a sefydliadau i helpu i ddatblygu eu gallu i ddiwallu anghenion preswylwyr lleol.
Roedd y gwasanaeth ar gael mewn nifer cyfyngedig o ardaloedd yng Nghastell-nedd Port Talbot yn flaenorol, ond bellach maent yn hygyrch i'r holl breswylwyr. Mae cyfanswm o chwe Chydlynydd Ardal Leol yn gweithredu dros yn y cymunedau yn y fwrdeistref sirol.
Meddai'r Cynghorydd Peter Richards, Aelod y Cabinet dros Ofal Cymdeithasol ac Iechyd i Oedolion,
"Gall ein cydlynwyr helpu pobl i helpu eu hunain gan ddefnyddio'u gwybodaeth leol a chysylltu pobl ag adnoddau sydd eisoes ar gael yn y gymuned. Rydym am gydweithio â phobl i gynyddu eu hannibyniaeth a'u gwydnwch personol fel y gallant fyw'r bywyd gorau posib.
"Mae Cydlynwyr Ardaloedd Lleol yn elfen allweddol er mwyn atal, oedi a lleihau'r galw am ofal cymdeithasol a gwasanaethau iechyd drud."
Meddai Emma Jones, un o'n chwe Chydlynydd Ardal Leol yng Nghastell-nedd Port Talbot,
"Mae cysylltu pobl â phobl yn rhan fawr o fy rôl. Efallai fod nifer o'r bobl rwy'n cwrdd â nhw'n teimlo'n ynysig ac yn unig. Rwy'n gweithio i ddod â phobl at ei gilydd mewn lleoedd lle mae ymdeimlad o gymuned, lle mae pobl yn cefnogi ei gilydd ac yn meithrin perthnasoedd hir ac ymddiriedus.
"Mae'n fraint i mi fod yn Gydlynydd Ardal Leol oherwydd gallaf helpu i ddatblygu cymunedau lle mae pobl am fyw ynddynt, lle maent yn teimlo'n ddiogel ac yn gysylltiedig."
