Neath Port Talbot Council is set to expand its Local Area Coordination scheme to be able to support more communities across the county borough.
The scheme is one of the Council’s priority programmes to prevent, delay and reduce the demand for costly social care or health services by supporting people to stay strong and to connect to existing resources in the community.
The pioneering community support scheme supports residents before they reach a crisis point to lead a better life and increase their independence and personal resilience. The scheme is open to people with disabilities or mental health needs, older and young adults, their families and carers.
The scheme’s outreach workers, known as Local Area Coordinators are embedded into local communities to act as a single point of contact. Using their local knowledge they provide information, advice, advocacy, local non service solutions and connections within community to help support people stay strong.
The Coordinators work closely with the person to focus on their strengths and build on their relationships with neighbours, friends and family.
Two additional Coordinators will be added to the scheme to cover the areas of Onllwyn, Seven Sisters, Crynant and Blaengwrach, and the areas of Bryn, Cwmavon and Port Talbot town.
Councillor Peter Richards, Cabinet Member for Adult Social Services and Health said:
“I am delighted that we are able to expand this successful scheme into more communities across Neath Port Talbot.
“With increasing pressure on social care and health services, it’s important that we use resources in the community to help improve people’s wellbeing so they can lead a better life.
“We want the county borough to be a place where everyone has the chance to get on in life and the Local Area Coordination scheme is one way of helping us to achieve this.”
Coordinator Emma Jones supported Geraint through the scheme on a one to one basis. Geraint said:
“I met the Local Area Coordinator when I was in a really dark place. She showed genuine interest in me, she wanted to listen to me and was not looking for faults but created opportunities by encouraging me to go out. She helped build my confidence and to take pride in myself. I have since started volunteering and looking for work because I was supported to see the possibilities ahead.”
The expansion aims to build on the success of the scheme that was launched in October 2015. To date the Coordinators have worked with 245 residents on a 1:1 basis to avoid accessing social services as a first option. The Coordinators have also built up close relationships with over 180 local businesses, organisations and community groups in order to develop the range of support available to residents.
There is no formal referral process to meet a Local Area Coordinator. People can be introduced in a number of ways, including through their local GP surgery, through friends and family or even their local shop.

Cyngor i ehangu cynllun cymunedol arloesol
Mae Cyngor Castell-nedd Port Talbot am ehangu ei gynllun Cydlynu Ardaloedd Lleol er mwyn cefnogi mwy o gymunedau ar draws y fwrdeistref sirol.
Mae'r cynllun yn un o raglenni blaenoriaeth y cyngor er mwyn atal, oedi a lleihau'r galw am ofal cymdeithasol drud neu wasanaethau iechyd drwy gefnogi pobl i gael mynediad i adnoddau sydd eisoes ar gael yn y gymuned.
Mae'r cynllun cefnogaeth gymunedol arloesol yn helpu preswylwyr cyn iddynt gyrraedd ymyl y dibyn er mwyn iddynt gael bywyd gwell ac i gynyddu eu hannibyniaeth. Mae'r cynllun yn agored i bobl ag anableddau neu anghenion iechyd meddwl, oedolion hyn ac ifanc, eu teuluoedd a gofalwyr.
Mae gweithwyr allgymorth y cynllun, a adwaenir fel Cydlynwyr Ardaloedd Lleol, wedi'u gwreiddio mewn cymunedau lleol i weithredu fel un pwynt cyswllt. Gan ddefnyddio'u gwybodaeth leol maent yn darparu gwybodaeth, cyngor a chefnogaeth i bobl fel gallant ddod o hyd i atebion i broblemau neu drwy gysylltu pobl â'i gilydd er mwyn iddynt helpu ei gilydd. Mae'r cydlynwyr yn gweithio'n agos gyda'r person er mwyn canolbwyntio ar ei gryfderau ac i adeiladu ar ei berthnasoedd â'i gymdogion, ei ffrindiau a'i deulu.
Bydd dau gydlynydd ychwanegol yn cael eu hychwanegu at y cynllun er mwyn cynyddu nifer y bobl y gellir eu cefnogi. Bydd un o'r cydlynwyr yn gwasanaethu ardaloedd Glyn-nedd, Onllwyn, Blaendulais, y Creunant a Blaengwrach, a bydd y llall yn gwasanaethu ardaloedd Bryn, Cwmafan a Phort Talbot.
Meddai'r Cynghorydd Peter Richards, Aelod y Cabinet dros Wasanaethau Cymdeithasol i Oedolion ac Iechyd,
"Ar ol gweld pa mor llwyddiannus yw'r cynllun ers iddo ddechrau, rwyf wrth fy modd ein bod yn gallu ehangu i gymunedau ar draws Castell-nedd Port Talbot. "Gyda'r pwysau cynyddol ar wasanaethau gofal cymdeithasol ac iechyd, mae'n bwysig ein bod yn defnyddio adnoddau yn y gymuned i helpu i wella lles er mwyn iddynt gael bywyd gwell.
"Rydym am i'r fwrdeistref sirol fod yn rhywle lle mae gan bawb gyfle i fynd ati i fyw eu bywyd ac mae'r cynllun Cydlynu Ardaloedd Lleol yn un ffordd o'n helpu i gyflawni hyn."
Roedd y Cydlynydd, Emma Jones, yn cefnogi Geraint trwy’r cynllun yn bersonol. Meddai Geraint,
“Cwrddais i â’r Cydlynydd Ardal Leol pan oeddwn mewn lle tywyll iawn. Dangosodd hi ddiddordeb go iawn ynof i, roedd hi am wrando arna’ i ac nid oedd hi’n chwilio am ddiffygion. Creodd hi gyfleoedd trwy fy annog i fynd allan. Helpodd hi i fagu fy hyder ac ymfalchïo ynof i fy hun. Ers hynny rwyf wedi dechrau gwirfoddoli a chwilio am waith oherwydd ces i fy nghefnogi i weld y posibiliadau o’m blaen.”
Nod yr ehangiad yw adeiladu ar lwyddiant y cynllun a lansiwyd ym mis Hydref 2015. Hyd yma mae'r cydlynwyr wedi gweithio gyda 247 o breswylwyr, gyda 47% ohonynt yn dychwelyd i gael eu cefnogi ar sail 1:1 yn hytrach na derbyn gwasanaethau cymdeithasol fel dewis cyntaf. Mae'r cydlynwyr hefyd wedi meithrin perthnasoedd agos gyda dros 180 o fusnesau lleol er mwyn ehangu'r amrywiaeth o gefnogaeth sydd ar gael i bobl yn y gymuned.
Nid oes unrhyw broses gyfeirio ffurfiol er mwyn cwrdd â Chydlynydd Ardaloedd Lleol. Gall pobl gael eu cyflwyno mewn sawl ffordd, gan gynnwys drwy eu meddygfa leol, drwy ffrindiau a theulu neu hyd yn oed eu siop leol.
